Hmm should've do collection of all hard coded keybinds? I've noticed some inventory actions are hard coded like dropping items (G) and there were some others also. Can't bind Inventory to good old G for that reason... Need to check later more if you aren't faster.
I don't think your 200m 300m 400m 500m image is really fair as there is more detailed background in the Reforger screenshots, especially the guys at 400m and 500m are standing in front of a bush! On the other hand the Arma3 screenshot shows them standing in a field that contrasts with their darker camo.
Some keys are bound with a 'Double' press, for example double tap Q to toggle lean left. If you attempt to rebind any key to double, it accepts the first key-press and binds to it

What I noticed that it seems to happen more often when selecting the spawn point by mouse eg. clicking the map marker.

No luck getting this to work so far. Initial attempt: forward & reverse keys did nothing, and vehicle was still attempting to reverse in response to some random axis's input. After trying to rebind W & S via the game UI, I managed to get the vehicle moving, but the handbrake was stuck on and could not be released, so movement is limited to a series of short jerks.
Being from the OFP days myself I have to agree, even though I have personally stopped using the feature, seeing it in there would carry on the legacy of the older titles. I say yay~
Can confirm. Ideally in-game rotation should be calibrated to match the 'in-game' rotation in TrackIR software (just like in Arma 3).
Actually I might have just found the conflict, I got forward to work now and heavily suspect the CharacterLeanVariable action to be the conflict. By default they show up as Q and E but they also don't seem to work by default, although that might be because CharacterLean is bound to the same key and just override the partial behavior. In any case, this is promising, but calls into question what the keybind UI should do when players bind keys to actions that block other actions.. hm, tricky. The least that is needed is for players to be able to clear default or custom bindings, so they can remove the partial lean keys.
Here is some good information I found in a US Army machine gun manual (TC 3-22.50 HEAVY MACHINE GUN M2 SERIES). The manual states that:

This issue is pervasive and, as far as I've seen, extremely widespread. Roughly half of all players I've talked to in my ≈40 hours of gameplay have said that this is a constant issue for them. It's making the game extremely irritating to play.
It looks like the answer is "Create New", and just accept the default path, because you can just ignore the 'project' after than and work on the Profile settings.
Hmm. When I open ArmA Reforger Tools I am presented with the Enfusion Workbench Launcher, and a blank list of projects, with options to "Create New" or "Add Existing". Which do I need to do in this case, and what path should I enter?

That's weird it did not work for me? I will try later today - if it actually already works like that, then this is irrelevant.
Just hold 'L' and the flash-light will toggle. This is how it works for me now.
Or by allowing to carry individual vehicle ammo boxes in hands (unable to use weapons unless dropped) physically - most immersive but probably requires more work.
Can confirm this is happening almost daily after an hour or two of play, possibly depending on how often you look at the map in game, since that seems to be the trigger.
Impossible to find extra bandages anywhere except on dead soldiers if they hadn't used it yet
As far as I know, short range radios are dual band radios. They receive and transmit on a set frequency (38 MHz in your case) and they always receive the default HQ frequency (48 MHz). This way every soldier can hear HQ messages having only his team's radio.
