Please make sure that all your drivers and software are up to date. Please check for any corrupted files and installations.
Delete the DayZ documents folder (make sure Steam cloud sync is disabled beforehand) and verify the game data through Steam.

It also doesn't seem to get removed when someone falls unconscious while bandaging. For instance, you can shoot an AI in the leg and then keep shooting until they fall unconscious while they try to bandage. You won't be able to bandage it, but you can verify in GM that they are still bleeding. This can even be reproduced without peer clients.

I don't quite understand why the memory LOD is so far off the visual LOD, but maybe as a workaround you could offer another parameter to set what LOD to use.
It would make more sense to use a visual LOD by default in my opinion, attachTo is a visual thing, but I can understand that you probably don't want to break existing code.

Could you explain how this check works on the client? Is it check only your player with IK animation or it also could check each player in a radius of loading? Also if weapon could be in Playerbase npc?
