@Benargee There's always a level of abstraction in ARMA. One could argue that ammo/fuel/repair trucks are unrealistic because there's nobody around to load crates, pump gas, or service trucks. Medical trucks and medical pods already allow the player to heal, so it would be fitting that medical buildings do as well. As for performance, it takes literally one line of code in Config.cpp (attendant = true;) to allow medical buildings to heal players, so I don't see how performance could possibly be impacted.
Issue resolved
I'm not talking about changing sides via scripting. I'm talking about changing sides using the groups feature of Zeus. Unless you're the admin of the server you're playing on, you won't be able to use that command.

I don't think the technical implementation is the problem. I think the problem is whether or not BI is willing to put in the time and money to make female assets with radio protocol and everything. Female models isn't a game-breaking issue. At the end of the day, it's another low-priority feature request. I wouldn't expect women to return to ARMA until ARMA 4.
The problem isn't that there aren't female models. The game is absolutely fine without females. Females neither add nor detract from the overall quality of the game. The problem is that there is an overall lack of official content in the game. For example, in ARMA 2 vanilla, there was a variety of civilian vehicles (ie. Skoda 120, VAZ Lada, Honda Civic, VW Golf, various trucks, pickups), but in ARMA 3 you only get a hatchback, an SUV, a pickup, and a truck. Female character models are just another thing missing from ARMA 3.
The community is always free to port content from ARMA 2 or make their own replacements, but until mods become more integrated with the game (i.e. Steam Workshop integration, auto-downloading) ARMA 3 will continue to feel lacking in content. Undoubtedly, BIS will add this content in the inevitable paid DLC to be released a year or two down the road.
While female soldiers are a contentious issue, female civilians should be in the game by the time the "Win" campaign DLC is released. Compared to ARMA 2, the amount of interactive assets in the game is meagre if not for the efforts of modders.

Late post but, I think that no particular emphasis should be placed on having female SOLDIERS ingame as women are a minority in the military.
Having said that female civilians however are not only an area that MUST be improved in ARMA 3, but must be able to do whatever male characters do. The reason for this is that female characters in ARMA 2 had absolutely no gameplay value whatsoever. They had the gameplay value of the mongrel in ARMA 2, which gives a whole new meaning to a certain word that begins with the letter B. If you give female characters the ability to wield and fire weapons, there's much less incentive for abusing the characters.
It's 9 days until launch and I really hope this is implemented.
