I particularly like the "attack only after visual contact" part. That makes a LOT of sense!
I don't see how this was definitely intentional server hopping. He may simply have logged back in at this point. The upsetting part for you is not the spawn, but the kill, right?
Allowing people to log back in like this, inside buildings or behind other players, is obviously something that needs to be improved though.
Not allowing to log in and out while inside buildings at all seems to me like the best suggestion. But you would have to be put back to where you were, not randomly moved to some other place.
Being seen before being able to see is unfair. Accidentally logging into danger yourself is as big a risk as having somebody log in on you.
Items disappearing is unfair as well. Right now, about half my logouts are not intentional but the result of server restarts or crashes. You can't punish people for that by taking all their progress away afterwards.

Weapon jamming on a Badly damaged weapon? Ok. Weapon jamming with damaged ammo? Also ok. Weapon jamming after firing pristine ammo from a pristine weapon? Absolutely not, please.
This is just like randomly sneezing, randomly falling off a slippery ladder or randomly losing items. They do happen in real life, but in every game, even in this one, the player needs to be in control. No random stuff may occur, unless the player is at least in some way responsible by getting sick or not keeping his equipment in good shape.
KOS sucks, but it happens because too many assholes and immature 12 year olds play the game (PEGI 18, anyone? Hello, parents?). These problems are in reality, not in the game. Fix them there.
I think stretching the interface depending on screen size shouldn't be too much of a problem. If all else fails, you can just make it smaller. An option for that is already in the game even now.

Not always, but when it happens, it usually happens several times over a short period of time.
Not specific to one server, and servers, on which it happens, may otherwise work fine and usually not have any such bugs.
Happens only after joining via the Steam overlay.
To get rid of the issue, I have to either join a different server, leave it and then join on a friend again (using the Steam overlay, this time it will work) or connect to my friends' server directly using its IP.
I did find one at a heli crash site yesterday, so they do still spawn there.
We found out that you can work around the bug by raising your weapon while crouched, before starting to move. Works with both the M4 and the AKs and with the Mosin as well, I believe. A friend said raising the weapon while moving worked for him as well, but it didn't for me, I had to stop first.
This happens with both AK types as well. It was introduced in 0.48, if I'm not mistaken.

Maybe on lower end machines, work a bit more with the overlay grass texture trick that is used for long distances anyway.
Or introduce several kinds of grass and take only certain kinds of it away on lower end machines, so that players with settings on high have some idea which of the grass they see may not be there for others.
If all else fails, force the player to render everything that is relevant for camouflage. People with insufficient hardware just wouldn't be able to play then. But obviously, this is a last resort and not suitable for an unoptimised alpha version.

GPU load goes to 100% immediately and stays there for as long as you look at a distant place. GPU temp increases accordingly, up to the point where safety circuits kick in, artifacts appear (pink blotches on ground, black lines along the 3D path of shadows) or the whole PC crashes. Didn't notice any impact on FPS outside the usual variance. CPU load slightly but noticeably decreases on 2 of 4 cores (by about 10%), the other two stay the same.
I've played the game with 2 Geforce GTX 570s (different models, DxDiag is for the latest, most stable one) and a Radeon HD 5750 so far, the issue is the same on all of them.
The issue seems dependant on zoom level, unzoomed scopes don't cause it all, zooming with eyes has a mild effect and looking through binoculars has a very pronounced effect. Looking at cities seems to have no greater or even a smaller effect than looking at fields and forests. Looking around in the distance has no different effect than looking at one place all the time. The bigger the area in the distance is, the bigger the effect is, regardless of how much of that area you can actually see through the binoculars without looking around. The biggest effect so far for me has been just outside Dubrovka, looking north east towards the gas station.
CPU load is usually 70%+-20% per core for me, GPU load is usually 40%+-10%.
If any sounds or visions were implemented that were purely "imaginary" to the player's character, then they should be illness related. As in, "I found some LSD. Yay, colours."
Playing random stuff (footsteps, zombies, animals, you name it) does resemble paranoia quite well, up to the point even, where the player has his character commit suicide to get rid of these sounds, because the game has become unplayable, just as life would become unbearable.
But: DayZ is a big sandbox. *Everything* is defined by the player, including what's right and wrong. If you want to get rid of KOS, you need to find a way to make people *want* to do something *else*, not punish them for what they do.
If you try to put some moral spin onto gameplay like this, then the logical next step after paranoia for murder would be increased health after helping somebody. Because you simply know it was the right thing to do.
I do think that making zombies both a more demanding and rewarding prey will take some pressure off players.
I have had this happen to me on top of the yellow construction crane. I was crouching in a corner, zoomed in with a scope, and pretty much aiming through the railing. I did not move at all when I died.
Same for shotgun as well, and you can't empty that either.

Obviously, how much colour, where exactly and under what lighting conditions needs to be tested, as it's probably a bit different for everybody's eyes. I would say, based on my experience, that there needs to be much less colour than in the image, but that there IS some colour remaining in a bright, moonlit night. And from there, it's a pretty seamless transition to black and white and finally to just black.
Same issue here. Seems to be server related, because it's much worse on some servers, but doesn't necessarily seem to be affected by the number of players. The servers where it's occuring generally seem laggy.
