It has been exactly one month with no comment at all on this issue. As this is a very serious issue from my viewpoint as the game, and more than likely BattleEye, is calling a function that it should not, then any/all updates are both welcome and warranted.
This 'feature' is specifically mentioned in the patch notes: "Melee: Cannot conduct melee attack from "lowered" melee stance (press space to chance stance to "raised")."
Same thing happened today on the experimental server with a screwdriver. I agree that the rate that items get damaged opening cans is too high.
Note: I just tested it again with the same result. I.e. I was unable to equip the pistol while prone. This time, after attempting to equip using the hotbar key, I did drag-and-drop the pistol in the inventory screen into the character's hands. Same result: namely it appears to equip the item, with associated character animations, only to de-equip the item.
You can edit your name in-game: when you first log on, with the ragdoll and menu options, click on the name listed in the lower-right corner. This will give you a cursor and you can edit your in-game as you desire.
After getting another pistol flashlight on a new character, I can report that it worked as of tonight. Note I did not turn it on using the 'L' keybind but by using the right-click menu option on the item itself.

It did have a 9V equipped (you would have noticed this if you looked at the attached image and the 'Detach 9V Battery' option). But thanks for the input.
Post-patch I had my character rolled back one week, then I played last night (Thursday) and did acquire a black CMF beret. When I logged on today I had a full character reset. Later that day, with the new character, I did again acquire a black CMF beret and after a server reset my character was fully reset again. Note, my server was not on the list of servers with connectivity issues and after two other server resets today my character was not reset.
Therefore, given what others have said, a beret may be causing issues. In addition, I've gotten error messages about beret on load-up and when first logging in. And in neither case did I have a shotgun.
I've also found you don't need to have your fists raised. I glitched through the concrete wall at Green Mountain simply by turning my mouse very quickly while running along the outside of the wall. In my experiences, quick mouse movements while moving near a solid object sometimes allow you to pass through said object.
I was able to launch the Launcher after using the registry entry file. And after doing some research, it appears that an anti-malware program may have deleted some legitimate .NET entries from the registry. Not sure why I haven't had an issue till now, but Launcher does work.
I've uploaded crash file(s) for five different crashes. Please let me know how to proceed.
