Unfortunately, with no particular penalty for server hopping at the moment, the only solution to re-looting the same area is having individualized loot tables, but that kinda ruins an important side of gameplay.

My card is fairly new (that is, in terms of it being used not in terms of generation), and the drivers are up to date. Are there any ideas from the more technical orientated on what could possibly cause this?
smaddeus I left my tech specs so that there was nothing left to think about. I am detail oriented so I figured I would leave as much info as I could instead of playing with the "need more info" tag.
I will attempt to reproduce this bug based off of the post above to see if the results are similar.
There is no shortage of collision issues at the moment.
I think this is well known by this point, I read somewhere that a dev said it was because all things that spawn in the game share the same sound database. That means a tree occasionally makes a gate sound, a bird makes a zombie sound, the wind makes the sound of a soda can.
However, I've noticed that the phantom sounds I hear happen to be sounds I legitimately heard recently.
Uploading an image of me exploiting it. You can really be unfair
I think what the game will inevitably need is a softcore and hardcore set of server.
Softcore: 3rd person enabled, server hopping, etc.
Hardcore: You lose gear if you swap servers, no 3rd person, etc.
The only problem is, what happens if the server you play on is full? You either wait or start fresh? That's not entirely fair, so I don't think the answers are so black and white. Without the ability to have multiple characters I think it might be somewhat difficult to enforce wipes on server hopping.
What we really need is a very basic service either in game to report these servers, or on this website to submit them.
Not exactly a bug, I think it is intended. That is, the getting knocked out by a shovel. Shovels however suck against zombies.
The bug I see here is that the ballistic helm doesn't protect against it which does not make much sense.
I think its fair that a melee hit from behind would KO someone in 1 hit, but I think in a face to face confrontation its reasonable to believe it would be hard to land a blow like that with a bat or shovel - thus maybe work that consideration into the game, and melee damage localized to the front takes more than one hit.
I have also noticed stopping whether raise or not when walking
Can confirm this. PvP loot is a race at the moment because you have until the player decides to respawn. When I die I usually wait a few minutes out of consideration but it is a bit unreasonable to expect players to adopt a universal code of conduct.
When a player dies, his/her body should remain until the server resets - or until someone hides the body. This way PvP has more secured significance. Perhaps this is intended, however, for the early stages of the Alpha to reduce item proliferation.

EDIT FOR MORE DETAIL:
Happens Day or Night.
Happens no matter what graphical sacrifices are made
**Way worse on a server that just booted with fresh loot.
