I've had 4 mysterious massive trauma events just like what was described by OP. Stairs have always been problem, but this isn't that. Twice I was running through the woods - screen turns red, player screams in pain, and shirt or pants go from pristine to ruined. It usually results in leg fractures too.
The last time it happened, I fixed myself up and stood there waiting to see if it was a sniper (I was doubtful), and nothing happened.
I think this is a bug that was introduced in .45 Stable. (I've got 600 hours in since SA launch).
I'm convinced that weapons have condition like the weapon accessories, but the game is bugged and we can't see them. I've had several Mosins now - all picked up from their spawn locations.
Some are deadly accurate with the pristine bipod+long range scope, and others shoot to the left. I was shooting just now at 300-400 meters and it was hitting left every time.
I think the weapons are working as intended, however we're not seeing their condition.

if you mean the ATC tower right on the runway, then I can confirm. Heading down the stairs from the glassed in area resulted in instant death for me. There was nobody near by and it was a silent death.
Stairs and ladders in general are glitchy.
This is a known issue - loot does not respawn in the current build. You need to search further inland, or look for freshly restarted servers, which will have lots of loot
well that is another bug :-)
I dunno - I like it at the moment. There a ton of M4s making them inaccurate makes sense for the most common gun in the game. All of this can be adjusted when there are more guns in the game. (and then the M4 can be rare again).
I think crappy inaccurate weapons should be more available than highly accurate modern weapons.
I hope this "bug" does not get fixed. bleeding happens very easily at first, and this seems like a good design choice. The player is encouraged to loot up - and that means they need to be very cautious at first. The player needs to eat a lot of food and water/drink at first to get to a healthy state. This is also a good design choice.
Currently in the inventory screen you can see different status:
Thirsty
Hungry
Bleeding
Healing Healthy
Sick
etc. Works fine for me.
Yes there are problems - no loot respawing at the moment. That will get addressed in a future patch.

Adding my vote to get this fixed ASAP. feels like a high priority item next to the remote exploit hacker bug and the vote admin bug.
Running through the forest, I could hear sodas being opened. I thought I was mishearing things, but the more it happened the more I was convinced it was happening.
I also had this happen. It was a terrible mic, but I heard a dog barking and the player moving his headset around. The mic was stuck on, and there was no way to communicate to the player to turn it off.
My first attempt to take a drink was from a well. The option to take a drink is there, however it doesn't seem to work. My character keeps complaining of being thirsty (within seconds of taking a drink). It isn't obvious if I need something else like a bottle, though intuitively, I should be able to drink as much as I want as long as I'm beside the well. I died about 10 minutes after this, I assume to dehydration (I had run from the coast to Guglovo)
I had a similar issue. I switched between my axe and my flashlight with the "1" and "2" keys. The first time I then clicked on the flashlight in the "tab" inventory screen in the Hands section. The next time I switched to my axe the flashlight disappeared from my inventory. I could not find it anywhere around me.
