Ate a rotten banana with flashlight equipped and powered on. Upon finishing eating, the flashlight was no longer in my hands or inventory. The beam could be seen projecting upward, on the ceiling as though it were dropped, but it could not be retrieved by looting.

I had a similar issue. I switched between my axe and my flashlight with the "1" and "2" keys. The first time I then clicked on the flashlight in the "tab" inventory screen in the Hands section. The next time I switched to my axe the flashlight disappeared from my inventory. I could not find it anywhere around me.
