In my previous game sessions (single player mission: "Elimination") the AI sometimes threw hand grenades in my direction. It could be more frequent but they do it.

The problem are the non team-players that wants to get the ultra-ghillie-sniper-grenade-rocket-launcher-kit straight from the beginning... just to get killed five seconds later. This behaviour drains resources like there is no tomorrow and causes massive resource problems. People should earn their stuff and that's what the conflict mode is for. If you know how to rank up fast you will get what you want.

I can understand that ragdolls might make it difficult to select the interaction points for the person healing the patient but to fix that, you could have a "Roll onto back" action to put them in the state they go into currently.

Oh, I haven't tried that.
But let's be honest, holding the F key would be the better and more intuitive solution to detonate the charges.
