Oh god yeah this happens, I was in a server today where this one guy somehow had global voip on constantly while he was on teamspeak... spamming the entire server constantly. I muted every player in the server and it did nothing.

whilst I have not has this issue with the flashlight it did happen to me with the gas lantern, it was working fine and lit up a huge area then the next time i used it i could see the light inside the lantern but it surrounding area was pitch black, perhaps this is an issue with light emitting objects and the game not registering that a lit object is in world?
Yep I get this also, whilst typing in chat your dude does a little chat dance when you type with, not game breaking but quite annoying.
I have a pristine lamp and a pristine large gas cylinder and the very first time i used it inside a house it lit the room like daylight.. it was glorious, then the very next time i used it nothing, I got the glow in the lamp like it was on but no light was thrown. I tried swapping gas and un-equipping then re-equipping the lantern to no avail, I even came across a worn lantern and tried that with no luck.
Confirmed at Rog, I didn't think there was any loot at all until my character ended up in the floor somehow and there was some loot under there, I couldn't move to loot it but it was there.
AHH I see, well that does explain it then.. so a zombie can smell you from 50M away? If that's true that gives this game a whole new level.. So when will antiperspirant become a loot item?
Yeah I have noticed this but unfortunately it doesn't happen every time, I have come around a corner many times to see a Zombie about 50-70 meters away facing the other direction, I stop instantly to check they haven't seen me and crouch backwards to circle around the other way behind the building but before I've even reached half way around the building the zombie has agro'd me seemingly through the building.. happens about 6-7 out every ten so a fair bit.
