I think the real issue is the terrible localization of this and other sounds. Hearing someone place an explosive or reload their rifle from 500 meters (or more) away is just terrible. As this issue has carried over from Arma 2, I don't expect it to be fixed, but ... it really needs to be fixed.

There is a direct correlation between this issue and collimited optics. It only occurs when a collimited optic is installed on the weapon, and appears to render where the projected aimpoint of the collimited sight would be.
It is only present when using a collimited sight. It generally follows weapon sway, as though it were a "ghost" of the projected aimpoint. Going ADS, it clearly merges with the projected aimpoint.
RCO - clearly visible
ARCO - slight flickering, but noticeable
ACO - very erratic movement and flickering
HOLO - least visible
It is not necessary to zoom in to see it, though it helps.
