У меня такое происходит почти постоянно (в 90% случаев) непосредственно при прогрузке персонажа на сервере.
Экспериментальным путем выяснил, что это можно исправить простым нажатием кнопки "Видео" (знаю, звучит бредово) в настройках игры: Esc - "Настройки" - "Видео". Вуаля :D

This happens close to 90% of the cases directly when loading a character on the server.
Fix by pressing "Video" in settings: Esc - Settings - Video.
