I reached top floor of Town house in Zelen, overlooking the store. Crouched by the window full health and not injured hungry or thirsty. Black screen saying I was dead.

My Character doesn't seem to require feeding or watering even after sprinting constantly for 30mins.

Happened after I was hit by player with fire axe, and I fixed blood.
