Same for me, happened a lot of time.

Last time was when the server restarted to apply the latest patch.
After the server came back online, i logged in as a fresh spawn...

This bug is seriously critical and hugely impact the gameplay.

I have the impression it occurs more often when you're in weapon raised mode.
