Here are the screencaps, tested in two locations (Kamyshowo and city north of NE airfiled that was added 2 patches ago), both are too narrow for a crawling men.
I've had a similar issue. Leg broken in a pub like building (only ground level, bar and taps for drinks). Crawling has a hitbox larger than a door frame which results in being stuck.

Maybe it's only an experimental issue.
Video for proof, quality is bad but all the relevant things are there.
I have same thing but I think it's a client side disconnect rather than a server going down. During my run from Electro->Cherno I've got disconnected 9 times and I still haven't reached Cherno. All of that on hardcore servers.
I have all kinds of characters on different servers. Including fully geared character that died few days BEFORE the patch hit. That one surprisingly is most consistent, I've had that one on at least 10+ servers
