Any building I've been in... if I somehow manage to gain zombie aggro they just come running through the wall as if it wasn't there. This has been my number 1 cause of death.

Mazzima nailed it. I've found that if you have VoIP open when you are killed, your mic remains on and goes global. Happened to two of my friends and another random guy.
