i noticed with a mod I am creating that sometimes items were getting duplicated, we noticed with Smoke grenades. http://youtu.be/IVBIbw-putA

Steps To Reproduce

use: player addMagazine "SmokeShell";
^adds one smoke grenade

use: player removeMagazine "SmokeShell";
^ removes one smoke grenade

use: player addMagazine "SmokeShell";
^ adds two smoke grenades

seems to be reproduceable 100% of the time
