Had the same issue. Status indicators didn't change after eating or drinking so i decided to relog, when i loaded back in i was fully energized, fully hydrated, healthy and red stuffed and my character vomited immediately.

This has happened to me also twice and one time with a friend while we were playing together. It seems to happen mostly when i'm near some kind of loot, either picking it up or trying to walk over it. The most recent time it happened i was on the second balcony in the new prison tower and tried to pick up and AK stock when i fell to the ground with a broken leg.
