Randomly happens when climbing tall ladders which you get off on the side of.
The movement breaks at the top of the ladder and makes you fall down the middle of the ladder resulting in death.

Climb tall ladders which require you to get off on the side of like the chimney stack ladders (Vybor industrial)
land_smokestack_brick object is the one I usually get this bug, but there are other buildings/ladders.

Over a year scheduled and still this is happening on multiple ladders, was this really fixed?

This is a known problem that is scheduled for a fix - scheduled for a fix meaning the fix has not been deployed yet or the issue has not been fully fixed.

I had a 2 hour life die on Skalisty island radio tower because I felt like having a scenic view..
Sure people are clumsy.. but..
Being able to catch the ladder while falling would be a great mechanic instead of just flailing arms....

If you could catch the ladder there would be no point in climbing or sliding down it, you could quickly just fall down and grab it making it the new Meta for getting down any ladder.
