First happened when trying to load game save after mission fail (Stepping Stone - Disintegration Point). Then every time I tried to resume the mission it crashed. The top green load bar would complete and then the mission load bar would look about done before crashing.
I ended up removing any mods that were on and reverting the mission to start over.
I tired uploading the report zip but it kept faily. Let me know another way to get you the file and I will.
This ticket can be closed. I am done with the SP campaign and am not interested in perusing the issue.

There was a hotfix yesterday do you still experience the crash? If so please upload your dumps here. Here is how to do that.
Per my previous post, I am no longer playing that campaign series. I am playing the Contact missions and have not had any crashes.
