I have experienced this two aswell. One time i somehow was able to get out the room. The other time i could open doors in the rooms next to me. Luckily this also alarmed a zombie which killed me so i could get a respawn.

I had the same problem. I solved it by dropping my bag on the floor and reequipped it and it appeared on my back.
