This bug has happed to me twice. Once with a Bag and once with a chest holster. I equiped the item and instead of going to the correct location it landed at my feet and did not register to be part of my inventory. I could run around in the game with the item at my feet and in the inventory screen you could see the item at my feet. If you vault while the item is at your feet it will correctly place the item where it is supposed to go. {F28494} {F28495}
