I have the same audio issue, tried reinstalling the game, made sure it was not my controller or my headphones, chat will work fine and then after an hour or two it just stops, I can't get it to restart no matter what. Different servers, have tried everything and it just happens everywhere. I can push the LB button and left keypad to change from whisper to yell with no problem but if I push left on the pad using the new controller scheme or even changing back to the old controller scheme the issue keeps coming up. Icon disappears and you cannot chat with other people in game.
