Having played this game since its first incarnation as Operation Flashpoint, I have observed the same failure in terms of hierarchical command structure throughout the evolution of this series.
I would like to see more strategic focus by incorporating a General and a team of advisers. This should make use of a top-down strategic command structure that encompasses aspects such as interception of intelligence, radar, etc. Group commanders should then follow the commands of the Generals and work to a common goal. There should also be a way to enforce the following of orders and not have people running off doing their own thing (i.e. replacing someone with the next in command, or an AI).
In short, if we are going to simulate war, it needs to be more than just a shoot-em-up.
