While in the aquatic level heading to shore there was a lone fish model that looked odd, half the model looked missing. Upon approaching the fish model loaded fully but edges were blocky.

Look for a large fish.

Most likely related to LOD(Level Of Detail) Mesh, which would explain the blocky appearance.
However it's obviously not working as intended, it shouldn't be that sensitive. I've had similar cases back in ArmA 2.
