If you die with a turned on radio, sometimes your voice will still be transmitted as a freshspawn, even if you no longer have a radio.
You won't hear the others on the radio but they can hear you. You don't know that you have that bug until somebody tells you. Relogging fixes it

This does not always happen but we've had the bug on a vanilla Chernarus server for multiple patches already
