of the many times I've had it happen I can't recall any specific pattern.
Usually after a multi hour session, with 4 hour server restarts.
Ping isn't an issue as I've had this while playing on my private (internal network) server with only 3 others.
One of the most memorable times, my gas mask bugged out (no mask overlay) and my player while aiming down sights would aim to the sky. which caused me to get beaten into a bleed which then was followed by the stuck blood indicator until I relogged.
I'm guessing this is the result of another bug that is not visual to the player until bleeding.

@Nate_LapT That is on a modded server. Results from that is unrealiable.
@Geez I am unsure whether this is something specific, I had this happen randomly twice now on Vanilla.
The hotbar dissapearing has not occured again.
@Cybrax Most likely was. I don't know specifically which server he was on but I could guess.
As this issue happens on Vanilla this isn't a one off issue. at least a few people have reported it, I've experienced it.
I wouldn't be surprised if this is related to the desync bug that causes the weapon in your hands to be out of sync.
@Geez Could the blood splat issue be an issue in timing during bandaging?
As in, the blood splat is between 2 animations and the bandaging finishes before the animation completes. which then stops the script in the blood splat code from checking if the player is bleeding/bandaged so the last state of the visual is left flagged enabled when it should be set disabled.
Basically the code is not looping back to uncheck enable visual and ends before looping when the bandage animation completes.
I encountered this today on 1.24 on a Vanilla community server.
- I had 3 bleeds from a zombie.
- Used a regular bandage to patch myself up. I held the button down so it fixed all 3 bleeds in one continuous action.
- Blood spatter remained on my screen.
- I logged off before my blood got to 100% so I can't say if that will clear the blood either.
