When will wound infection be re-implemented? This used to be an important part of medical gameplay - and was the primary reason the alcoholic tincture existed in the first place. The tincture serves barely any purpose at all without it.
The Medical Attention Updated mod has it working well (effects are periodic blurry vision, fever and hydration loss), you can even customise the risk of contracting it. In a vanilla implementation I would expect different sources of the injury to affect the risk. Dirty zombie hands - very great. Pristine kitchen knife - very small. And everything in between...
