I had a similar experience climbing a very high brick chimney-like tower, but I wasn't able to remember my exact location. Once at the top, it wasn't climbing on the ladder and eventually fell to my death.

Similar thing happened to me in Novodmitrovsk. There is a smokestack near the center of town. When you climb up it, your character gets further and further away from the ladder. By the time you are at the top, you are too far to exit the ladder on the platform and you fall to your death.
