I've been playing on experimental servers and noticed that while using items from your inventory there is a chance that the icon of the used item still remains in the same spot in the inventory when used. Most commonly observed with food items that should be removed after fully consumed. A way to remove the icons is to place the piece of clothing on the ground and equip it again, which seems to cause the icons to reset to the correct way of showing what items are in the piece of clothing.
