I received the "You are dead" screen immediately after getting off of a transit bus. One aspect that might be important is that I was holding down the "W" key during the animation. While the "You are dead" screen was showing, I noticed I did not have the ability to respawn as a new character. I chose the option to exit the server, instead. When I reconnected to the same server, my character was till alive, albeit a few hundred meters away from the location where I left the bus.
