Not sure how to reproduce surely, it appeared kind of random. But at least 50% chance to happen it seemed

I agree with this, it's real problem if you're using cover and beign shot at.

Unfortunately we were unable to reproduce the issue you have described. This has probably been caused by the various desync issues we are aware of and they have been scheduled for a fix.
