So I had the black screen saying "You are dead" but my character kept saying he was hungry and I could access my inventory. When you are really dead you cannot. I waited around 3 hours and my character of dehydration. After my inventory was no longer accessible the respawn button finally worked and I could start a new game.

Almost 24 hours later problem persists, tried loggin in on servers that say that my character will be deleted but I sill spawn under water with the same character.

Also happened to me. I was inside a cubicle alone without any windows and all of a sudden got both my legs broken and had to crawl.
