This was happening to me as well, until I found the cause; the person is using ALT-F4 to exit the game so you can't get their loot. I tested this with my friend to confirm.

This is constantly happening to me as well. I spent hours getting some good gear, switched server a few times due to hostile people and laggy servers and found that I had my character reset as I logged on to a new server.
