I changed my keyboard which seems to have fixed the issue. My previous keyboard was a Corsair K70 which has all sorts of media shortcuts and a USB pass-through (which I was not using) but my guess is it was something to do with power usage or the USB. Either way, I moved to tenkeyless Logitech which has solved the issue. Such a shame though because I loved the K70.
