The video memory fix worked for me as well, still get the odd performance issue, worth a try.

Im getting some really bad performance, to the point the game is unplayable, i run the game as low as it can go almost, and up to this point it was pretty ordinary to look at but it ran smoothly enough and very playable, but when it drops to a crawl that seems like every few minutes im toast if there are players or zeds around, now im cool with the alpha testing bit and dont mind the odd step backwards, but the bloody thing has to be playable to some extent...

Yeah same thing happening to me, updated my Nvidia drivers not so long ago, but completely unplayable, strangely the first 3-5 mins it plays like im on a Cray and then it all goes to s%#t, im out until this is fixed....
