It just happened in-game and I have yet to try to reproduce, but I came across a stranger and he wanted to try out putting a burlap sack over my head. He put it over my head, but then was unable to find a prompt to remove the sack. I opened up my inventory and dragged the sack off my head, but my screen stayed black after it left. He said he could see my player fine with no sack on, and it disappeared from my inventory. He says it was floating in the air but was unable to be interacted with. He had to kill me because after removing the burlap sack the screen stayed black.
