I was at full health, food and thirst, a zombie tapped me lightly once from 3 or 4 meters away and I died. I had been in lots of similar combat before that with no problem and it should have taken many more hits to kill me. There was no "unconscious" just dead. There is no cause-of-death message to verify what happened.

This all happened at the factory west of Orlovets. About 20 yards inside and to the left of the only entrance around the outer wall, one of two zombies that spawned in that area did it.

Respawn did not seem to work normally afterwards. After the first time I hit respawn, it gave me a waiting message then just said dead again. I logged out and in a couple times, still dead. I was able to respawn by switching servers and was able to log into the old server alive afterwards.
