This occurs when the player has their flashlight in their hand and is active. If the play goes into their inventory screen and "drops" the flashlight in their vicinity the flashlight will still remain "on" in their hands as well as the vicinity. The flashlight will remain in this position until the flashlight is removed from the players hands.
