I don't know exactly how it's caused either by walking normally with shift. But I get stuck in a permanent walking mode. I can still run when double tapping "w" but shift doesn't return you to the normal jog mode. You have to log out and log back in to fix it.

I can confirm that this occurs. Double tapping shift does not fix the issue. I have heard on Reddit that pressing the right mouse button can fix the issue, but that isn't really the point. I think the game thinks you are aiming a weapon (so you can't run), but without any weapon in your hands.

from my experience double tapping shift seems to toggle walk sometimes,
I have found this happens to me where its not toggle usually after sprinting and walking up stairs into a house so it could also be a fatigue thing....
