When walking to the top floor of a high rise, I climbed the ladders on to the roof. Instead of putting you on the roof, you come off the ladders in the hole/gap/doorway. This causes you to fall back down to the bottom of the ladies and take damage.

Steps To Reproduce
Tried four times, died after the second time. On the fourth attempt I crouched first then climbed stairs which seemed to put me on the roof.

I also had this issue, I think it broke my legs on around the 3rd try but after testing my blood I seemed fine again!
