In the mission Damage Control near the end, the aircraft flying-by crashes & collides with the chopper flying-by. I've tested it twice & got the same results.

Steps To Reproduce
- Go to Campaign
- Expand Prologue
- Play mission Damage Control
- Play until the ending cut-scene where you are talking to Adams
- Look to the right of you & watch as the aircraft collide with the 2 choppers
