Too many context menu buttons for the X360 controller

Description

Currently there are 3 context menu buttons (A, D-Pad UP, D-Pad Down), which seems a bit excessive in the default X360 controller scheme.

Maybe get rid of the A option, so that we can only use the D-Pad, sticking with the scroll wheel theme?

Mass closing ancient tickets with no activity for > 12 months; assume fixed or too trivial.
