VON and Radio not using "Default Communications Device"

Description

When using a headset, the headset is the Windows "Default Communications Device". The game is not using the Default Communications Device for the microphone or the ear piece. The Radio and VON audio channel is incorrectly mixed into the rest of the game's surround sound, leaving the headset's ear piece silent. This defeats the purpose of using a headset.

Voice Over Net and Radio chatter should be on a separate audio channel. This channel should output to and take input from the Default Communications Device (if available) this way only the Radio and VON audio is going to the headset and the rest of the surround sound goes to the regular default output.

Solution: Add the ability to select separate output devices for Radio/VON and Game. Add the ability to select input device for VON. Auto-detect these based on Windows settings.

Additional Information

I am using a wired Xbox 360 controller with a wired Xbox 360 headset plugged into the controller. The headset is seen by Windows as a USB headset.
