Description

If you try to create a report with an attachment bigger than 5mb it will show an error but still creates the issue.

Steps To Reproduce

Click report issue
Fill the form and select a file bigger than 5mb
press submit report
after the error press 'back' on the browser
change the attachment to a smaller file
press submit report
now there are two identical reports one with the attachment and one without.
