not sure, it happens at random times

Additional Information

i figured out to exit i would press v to vault then quickly go prone, and that seems to fix the problem

Yep, I've had this problem myself as well as seen others experience it. Very hard to replicate, however. Seems to be related to hitting some kind of combination of movement key presses whilst running.
