While in a gun fight, if you get knocked out it can cause a round to not be chambered, after waking up and retrieving your gun you will not be able to re-chamber a new round and the magazine would not be effected.

Pick gun up
If it has no round chambered attempt to chamber a new round and it should not allow you despite the sound and animation completing and playing.
