Updating the Windows
Updating video card drivers
Rollback video card drivers
Check SSD and RAM
Check free space on SSD
Reinstall game and mod with deleting all game and mod folders
Create another local Windows account
Create another profile in the game
Check SFC scannow
Connecting to another server without mods - OK
Clearing Steam cache
Check GPU and CPU temperature

secExt_x64.dll That comes from the @Rocket Life modpack. Ask them. It failed trying to get your hard drives serial numbers.
Also you are loading the DiscordRichPresence mod twice, that's probably not good.
