The animation for the sneak attack with the sickle is the same as with a knife. The issue is that the weapon is curved, so you end up stabbing the infected with the curved outside of the weapon.

I suggest changing the animation to more realistically apply the attack with the sickle or make it so the sickle can't be used for sneak attacks.
