I had bug where I took the magazine out of my CR 527 mid fight to reload. Then I reloaded and got the animation but the there was just 1 bullet in the chamber and the wasnt reloaded into the gun. I recorded it if that helps. It happend on Livonia 4295 official.

I didnt had time to check that before I died. But when this is a known issue to you, it probaly was like that.
